Lyn Baluyot CEO

Lyn is a seasoned healthcare leader with over 25 years of experience as a nurse, administrator and consultant.  As a dynamic healthcare executive, Lyn has led global enterprise-wide complex clinical transformations attaining sustainable results. Her collaborative style enables her to work with multiple different stakeholders to achieve transformational change with sustainable results.  Her deep operational experience enables her to work with other health system partners to shape and achieve the vision for future healthcare delivery while sustaining financial efficiency, improving quality and enhancing the patient and family experience.

After almost 20 years as a consultant with Deloitte US and Deloitte Canada, Lyn joined TransForm in 2018 and led the Clinical Systems and Transformation office at TransForm and was responsible for all clinical systems including Cerner throughout the Erie St. Clair region, as well as any ongoing transformation programs.  She successfully led the most comprehensive clinical transformation program (e-VOLVE)-within the former Erie St. Clair region with her hospital partners resulting in the successful implementation of the Cerner Millennium hospital information system across four hospital organizations in big bang style during probably the most trying time of healthcare:  the COVID-19 Pandemic.  The e-VOLVE program has resulted in a longitudinal, contiguous patient record for all patients being served in Windsor-Essex and Chatham-Kent that can be readily accessed by any provider at any of the four hospital organizations as well as the 4 hospitals achieving EMRAM 6.  She is currently co-chair of the Cerner Provincial Collaborative representing 10 instances of Cerner, 58 hospitals.  In April 2022, she was promoted to the position of Chief Executive Officer of TransForm where she continues to work with not only the 5 hospitals in Erie St. Clair and the 3 Ontario Health Teams to deliver Information Technology and Supply Chain Services.  

Dr. Antonio Meriano Chief Medical Information Officer

Dr. Meriano is a practicing Emergency Physician with over 20 years of Clinical Emergency Medicine experience. He has been TransForm’s Chief Medical Information Officer since 2013. During that time, he has assisted with the implementation of a regional Hospital Information System with 4 hospitals in Southern Ontario. Dr. Meriano served on the team working on the Cerner Provincial Resource Model (CPRM). This group worked on standard reference model of Cerner incorporating some of the best features from all the provincial instances. He also successfully completed Cerner’s Physician architect program. He has extensive experience with clinical technology and considers himself an early adopter of technology.

David Putnam Director Information Technology & Chief Information Security Officer

As the Director of IT/IS – Chief Information Security Officer (CISO), David Putnam is responsible and accountable for leading Transform and member hospitals Information Security Program. David has more than 3 decades of progressive IT and information security experience in small, medium, and large health care, Fintech and Telecom enterprises. His expertise includes developing and implementing enterprise security programs to enhance a company’s security posture, to manage enterprise risks and compliance, and to support revenue generation and growth.

Prior to joining Transform, David, held many positions at Blue Cross Blue Shield of Michigan.  He has well-rounded experience in information security governance, risk and compliance, security audits, business continuity and technical infrastructure support, security operations, risk management and incident response.

David is a Certified Information System Security Professional (CISSP), Certified Ethical Hacker (CEH) and previously held multiple Cisco and Checkpoint certifications.

Megan VanderEnde Director, Finance

Megan VanderEnde joined TransForm’s leadership team in 2021. She has been a part of the Finance team at TransForm since 2015; prior to which she worked in both the manufacturing and agriculture sectors. Megan is a Chartered Professional Accountant (CPA) and uses her skills to serve her community as Board Chair for Habitat for Humanity Chatham-Kent and as a mentor for CPA Ontario.

Michelle Watters Director of Stakeholder Relations, People and Performance

Michelle Watters joined the TransForm team in October 2013. Most recently, Michelle was the Chief Operating Officer and Executive Director for AUTO21, a national automotive research initiative supported by the federal government. Prior to her employment with AUTO21, Michelle spent nine years at National Health Partners/KMG Health Partners in Toronto and in the United Kingdom, working as the Senior Manager of Disability Services.
