Lyn Baluyot CEO

Lyn is a seasoned healthcare leader with over 25 years of experience as a nurse, administrator and consultant. As a dynamic healthcare executive, Lyn has led global enterprise-wide complex clinical transformations attaining sustainable results. Her collaborative style enables her to work with multiple different stakeholders to achieve transformational change with sustainable results. Her deep operational experience enables her to work with other health system partners to shape and achieve the vision for future healthcare delivery while sustaining financial efficiency, improving quality and enhancing the patient and family experience.

After almost 20 years as a consultant with Deloitte US and Deloitte Canada, Lyn joined TransForm in 2018 and led the Clinical Systems and Transformation office at TransForm and was responsible for all clinical systems including Cerner throughout the Erie St. Clair region, as well as any ongoing transformation programs. She successfully led the most comprehensive clinical transformation program (e-VOLVE)-within the former Erie St. Clair region with her hospital partners resulting in the successful implementation of the Cerner Millennium hospital information system across four hospital organizations in big bang style during probably the most trying time of healthcare: the COVID-19 Pandemic. The e-VOLVE program has resulted in a longitudinal, contiguous patient record for all patients being served in Windsor-Essex and Chatham-Kent that can be readily accessed by any provider at any of the four hospital organizations as well as the 4 hospitals achieving EMRAM 6. She is currently co-chair of the Cerner Provincial Collaborative representing 10 instances of Cerner, 58 hospitals. In April 2022, she was promoted to the position of Chief Executive Officer of TransForm where she continues to work with not only the 5 hospitals in Erie St. Clair and the 3 Ontario Health Teams to deliver Information Technology and Supply Chain Services.

Mina Girges Vice President & Chief Information Officer

Mina Girges is an accomplished IT executive with over 15 years of experience leading enterprise technology strategy, digital transformation, and operational modernization. With a strong background in the automotive and private equity sectors, Mina brings a results-driven and innovation-focused approach to healthcare technology leadership.

Prior to joining TransForm, Mina held senior leadership roles at ABC Technologies, a global automotive supplier, where he served as Senior Director of IT Operations. In this role, he led large-scale infrastructure and cybersecurity transformations, delivered multimillion-dollar cost savings, and played a key leadership role in several successful merger and acquisition integrations. His ability to align IT strategy with business outcomes enabled system-wide improvements across global operations.

As Vice President and Chief Information Officer at TransForm Shared Service Organization, Mina is responsible for leading the modernization of IT systems and infrastructure in support of five hospital partners and other health service providers across the Erie St. Clair region. His mandate includes strengthening cybersecurity, advancing cloud capabilities, and delivering scalable, secure technology services that enable better care and improved operational efficiency.

Mina is known for his strategic mindset and collaborative leadership style. He works closely with stakeholders to ensure technology investments support equity, care delivery, drive innovation, and meet the evolving needs of healthcare organizations. Drawing on his cross-industry experience, he brings new thinking and best practices to complex environments.

Mina holds an MBA from the University of Windsor, a Bachelor of Commerce, and certifications in Data Science and Operations Management (McGill University). He has also been recognized as one of MoldMaking Technology Magazine’s “30 Under 30” for his contributions to innovation in technology leadership.

As Vice President and CIO, Mina plays a key role in advancing TransForm’s digital health vision—building a resilient, future-ready IT foundation that supports high-quality care, regional collaboration, and long-term system sustainability.

Dr. Antonio Meriano Chief Medical Information Officer

Dr. Meriano is a practicing Emergency Physician with over 20 years of Clinical Emergency Medicine experience. He has been TransForm’s Chief Medical Information Officer since 2013. During that time, he has assisted with the implementation of a regional Hospital Information System with 4 hospitals in Southern Ontario. Dr. Meriano served on the team working on the Cerner Provincial Resource Model (CPRM). This group worked on standard reference model of Cerner incorporating some of the best features from all the provincial instances. He also successfully completed Cerner’s Physician architect program. He has extensive experience with clinical technology and considers himself an early adopter of technology.

Kim Ramirez Digital Health Services & Support Hub

Kim Ramirez is a seasoned healthcare leader with a strong clinical foundation and over a decade of experience driving digital health transformation. As the Director of Applications, Integration, and Development for TransForm Shared Service Organization, Kim plays a pivotal role in advancing health system innovation and efficiency for five hospitals in Southwestern Ontario.

With her extensive background as a registered nurse and her expertise in health informatics, Kim has led several transformative projects, including the successful implementation of the Oracle Health Millennium Hospital Information System (HIS) across the Erie St. Clair region. This ambitious initiative created a unified digital infrastructure, enhancing care delivery and operational effectiveness across all partner hospitals.

Kim holds a degree in Health Informatics from McMaster University, is HITS-certified, and is an accomplished graduate of the Dorothy Wylie Health Leaders Institute. Her dedication to advancing digital health earned her the prestigious title of Digital Health Canada’s 2021 Digital Health Leader of the Year.

A passionate advocate for fostering the next generation of digital health leaders, Kim shares her knowledge and expertise as an instructor for the University of Windsor’s Health Informatics Certificate Program. Through her work, Kim remains committed to improving healthcare delivery and empowering clinicians with innovative digital tools to provide better patient care.

Megan VanderEnde Director/Chief Financial Officer

Megan VanderEnde joined TransForm’s leadership team in 2021, bringing with her a strong foundation in finance and a diverse background in the manufacturing and agriculture sectors. She has been a valued member of TransForm’s Finance team since 2015.

In 2024, Megan led the successful implementation of an Enterprise Project Management Office, enhancing TransForm’s ability to plan and monitor digital and capital projects in close collaboration with hospital and provincial partners.

A Chartered Professional Accountant (CPA), Megan is deeply committed to community service. She currently serves as Board Chair for Habitat for Humanity Chatham-Kent and mentors aspiring CPAs through CPA Ontario.

Michelle Watters Director/Chief Human Resources Officer

Michelle Watters is Chief Human Resources Officer and Director, Stakeholder Relations at TransForm Shared Service Organization. With more than 20 years of executive leadership experience, she oversees Human Resources, Communications, Stakeholder Relations, and Digital Health initiatives while advancing organizational performance, workforce excellence, and strategic partnerships.

Michelle is recognized for her leadership in organizational transformation, enterprise risk management, business continuity, change management, and crisis communications, including guiding workforce and stakeholder response efforts during significant events.

An experienced governance leader, she has served on the St. Clair College Board of Governors, the Windsor-Essex County Health Unit Board, the Workplace Safety and Insurance Appeals Tribunal, and the Ontario Centre of Innovation’s College of Reviewers.

Michelle holds master’s degrees in Rehabilitation Counseling and Educational Administration and is a member of HRPA, a CHRE candidate, a Fellow of the Certified Vocational Rehabilitation Professional designation, and a Registered Rehabilitation Professional. She also holds certificates in ProSci Change Management and Performance Management.

Aaron Lapczuk Director of Applications, Integration and Development

Aaron Lapczuk is the Director of Applications, Integration and Development at TransForm Shared Service Organization (TSSO), where he provides strategic leadership for multidisciplinary teams responsible for clinical and business applications, clinical informatics, business analysis, interoperability, software development, and digital health solutions that support healthcare organizations across the Erie St. Clair region.

With more than 20 years of progressive leadership experience in healthcare technology and digital transformation, Aaron has worked collaboratively with clinicians, operational leaders, and technology teams to deliver innovative solutions that improve patient care, strengthen regional collaboration, and support the evolving needs of the healthcare system.

Aaron is passionate about leveraging technology to enhance clinical and operational outcomes, improve the user experience, and enable more efficient, connected models of care. His leadership has contributed to numerous regional initiatives, including enterprise electronic health record implementations, complex solution deployments, major system upgrades, the successful migration of Oracle Health Millennium to Oracle Cloud Infrastructure (OCI), and the adoption of innovative technologies that strengthen clinical and operational capabilities.
