Lyn Baluyot is the newest member of TransForm’s senior leadership team. She has over 20 years of experience in the healthcare industry, most recently as Partner: Health Transformation, National Lead-Health Activation Services at Deloitte Inc. in Toronto. She has also led several large scale transformation projects across multiple academic health science centers, tertiary, and community hospitals within North America.
Vice President, Chief Information, Privacy & Security Officer (CIPSO)
Mazen Joukhadar joined TransForm's senior leadership team in November 2015. He is a past partner with Accenture, a global management and IT consulting company where he led a division of over 80 employees in North America and was accountable for over 1,000 employees internationally. He has also worked across several industries, including public health and insurance sectors. Maz's primary focus was delivering strategic solutions to support a client's customer relationship management vision. Maz's career evolved over his 15+ years at Accenture, starting as a technical architect and eventually focusing on strategic consulting, developing business plans and IT roadmaps.
Dr. Antonio Meriano
Chief Medical Information Officer
Dr. Meriano is a practicing Emergency Physician with over 20 years of Clinical Emergency Medicine experience. He has been TransForm’s Chief Medical Information Officer since 2013. During that time, he has assisted with the implementation of a regional Hospital Information System with 4 hospitals in Southern Ontario. Dr. Meriano served on the team working on the Cerner Provincial Resource Model (CPRM). This group worked on standard reference model of Cerner incorporating some of the best features from all the provincial instances. He also successfully completed Cerner’s Physician architect program. He has extensive experience with clinical technology and considers himself an early adopter of technology.
Vice President, Business Development
Derek Robertson came to TransForm from Selectcore Ltd., where he held multiple executive positions, including Vice President of Compliance and Vice President of Operations. Prior to Selectcore Ltd., Derek worked as Vice President of Customer Service and Carrier Relations at Canquest Communications in Chatham. Currently, he serves as the Vice-Chair of the Lambton Kent District School Board after serving two terms as a Councillor for the Municipality of Chatham-Kent.
Megan VanderEnde joined TransForm’s leadership team in 2021. She has been a part of the Finance team at TransForm since 2015; prior to which she worked in both the manufacturing and agriculture sectors. Megan is a Chartered Professional Accountant (CPA) and uses her skills to serve her community as Board Chair for Habitat for Humanity Chatham-Kent and as a mentor for CPA Ontario.
Director of Stakeholder Relations, People and Performance
Michelle Watters joined the TransForm team in October 2013. Most recently, Michelle was the Chief Operating Officer and Executive Director for AUTO21, a national automotive research initiative supported by the federal government. Prior to her employment with AUTO21, Michelle spent nine years at National Health Partners/KMG Health Partners in Toronto and in the United Kingdom, working as the Senior Manager of Disability Services.
Senior Executive Assistant and Governance Coordinator
Karen Montminy joined the team in July 2013 and brings over 19 years of healthcare experience to TransForm. Karen joined TransForm from Chatham Kent Health Alliance (CKHA) with extensive executive support background and experience in Board activities within the healthcare environment.
