TransForm is committed to delivering exceptional service to its member hospitals and other customers. Each day, our employees come to work ready to help. To recognize the effort of employees, TransForm has two recognition programs.

Cheers to Peers recognizes employees for professional and personal accomplishments.

Sometimes, employees go the extra mile and deliver a wow!

To help recognize these instances, TransForm introduced the Going the Extra Mile recognition program for TransForm employees. Hospital members, TransForm employees and other customers can submit an online nomination based on the criteria below to recognize employees who have “gone the extra mile” in providing services.


Nominations must clearly demonstrate how the individual exceeded customer expectations and went beyond their regular duties related to the provision of exceptional service within one or more of the following pillars:

  • Solution Delivery
  • Communication
  • Accountability
  • Responsiveness and Professionalism

Each Going the Extra Mile recipient will receive:

  • A Letter of Achievement
  • A copy of the nomination outlining the details of their efforts
  • A ballot to be entered into a draw for a quarterly prize
  • Recognition in the monthly internal newsletter – TransForum
  • Recognition in the quarterly external newsletter – TransForm News
  • Recognition at the TransForm Town Hall

There is no limit to the number of nominations an employee can receive.

Nominations that commend employees for a job well done rather than going beyond their regular work duties are referred to Cheers to Peers.
