Press Release: Patient Transitions from Hospitals to Long-Term Care Take a Major Step Forward
Southwestern Ontario, November 30, 2023 – TransForm Shared Service Organization, in collaboration with Project AMPLIFI, marks a significant milestone in healthcare integration with the successful implementation of two-way patient data exchange between hospitals and Long-Term Care homes. This milestone represents go-lives for 36 hospital sites across 20 hospital organizations. Known as the Ontario eHub initiative, these concerted efforts have resulted in an expansion of enhanced connectivity for improved patient care coordination and streamlined workflows for clinicians. Hospital sites will be able to exchange data with each other as well as more than 175 long-term care homes that are already connected.
These go-lives are a pivotal step in advancing the interoperability of healthcare data, allowing seamless access to up-to-date patient health records, and facilitating efficient information exchange among healthcare providers. This two-way data exchange enables hospital healthcare providers to quickly access and integrate patient health records from diverse healthcare settings, minimizing the time and resources traditionally expended on manual data transfer methods.

The Ontario eHub Health Information Exchange (HIE), under the auspices of Project AMPLIFI, is part of the Ministry of Health’s initiative to bolster clinical information exchange between Oracle hospitals. TransForm spearheads the project management oversight for 63 hospitals utilizing the Oracle Health Information System (HIS) as part of Project AMPLIFI for Oracle Health hospitals.
This accomplishment brings us closer to the overarching goal of creating a more patient-centric healthcare landscape by minimizing redundancies in medical history sharing, improving care coordination, and reducing readmissions. Patients across diverse care settings will benefit from a higher standard of care, supported by streamlined data accessibility and enhanced collaboration among healthcare providers.
